Eyelid Deformities and Eyelid Tumors – Causes and Treatment

What are eyelid deformities?

Our eyelids serve an essential function – they protect our eyes. When they are no longer in their proper position, they cannot perform their protective function, which can impair vision or lead to inflammation. There are different eyelid deformities, each with its own causes. Eyelid deformities such as ptosis, drooping eyelids, entropion, or ectropion can be corrected through minor surgical procedures in eyelid surgery.

Definition
Entropion is a condition characterized by the inward rolling of the lower eyelid with a loose edge. The causes often involve age-related sagging, but in some cases, scarring can lead to shortened conjunctiva.

Symptoms
Due to the inward-turned eyelid edge, eyelashes may rub against the cornea and conjunctiva, leading to chronic tearing, discomfort, and, in the worst cases, small scars on the cornea that can result in vision impairment.

Treatment
Treatment involves tightening the eyelid to restore proper tension. The procedure is performed through small incisions at the inner and outer corners of the eye, which typically heal without visible scars. After the surgery, the eyelids may be swollen and slightly bruised for about a week. Cold compresses are initially helpful for reducing swelling, and after a few days, warm compresses may be more suitable. Everyday activities can be resumed without restrictions, although it’s advisable to avoid vigorous physical activity for around two weeks.

Definition
Ptosis refers to the permanent drooping of the upper eyelid. The causes of ptosis can be attributed to a congenital or acquired malfunction in the eyelid’s lifting mechanism.

Symptoms
Ptosis results in a restricted field of vision because the upper eyelid partially or, depending on the severity of ptosis, significantly obscures the upper part of the eye.

Treatment
Treatment for ptosis often involves surgically shortening the tendon attachment. This is typically done through a small incision on the upper eyelid.

After the surgery, there may be swelling and bruising of the eyelid for one to two weeks. Cold compresses in the initial phase, followed by warm compresses, can aid in wound healing and reduce swelling. Everyday activities can be resumed as usual, though it is advisable to avoid vigorous physical activity for one to two weeks.

Definition
Ectropion refers to the outward turning of the eyelid margin, away from the eyeball. In most cases, it is caused by age-related skin laxity, and less commonly by shortened skin of the lower eyelid due to preexisting scars.

Symptoms
Ectropion leads to chronic tearing, a scratching sensation, and can result in visual impairment due to the development of corneal scars.

Treatment
Treatment for ectropion involves tightening the eyelid to restore the proper lid tension. The surgery is performed through small incisions in the inner and outer corners of the eye, which typically heal without visible scars. After the procedure, there is usually swelling and some bruising around the eyelids for approximately one week. To reduce swelling, cold compresses are recommended initially, followed by warm compresses after a few days. Everyday activities can be resumed as usual, though it is advisable to avoid vigorous physical activity for about two weeks.

Dermatochalasis is the medical term for the condition commonly known as droopy or sagging eyelids. It is typically characterized by an excess of loose skin that hangs over the upper eyelids, sometimes extending down to the eyelash line or further.

Dermatochalasis can be a result of aging or have a genetic basis.

Symptoms
Depending on the severity, dermatochalasis can lead to a sensation of heaviness on the upper eyelids, and in more extreme cases, it may restrict the field of vision. Droopy eyelids can also contribute to a tired or aged appearance.

Treatment
The treatment for dermatochalasis involves the surgical removal of the excess skin of the eyelids and the underlying tissue.

Following the procedure, the field of vision is restored, and the eyes appear larger and more vibrant. Expect some swelling and bruising around the eyelids for about one to two weeks after the surgery. Initially, cold compresses are recommended, followed by warm compresses to aid in the healing process and reduce swelling. Everyday activities can be resumed as usual, although it’s advisable to refrain from strenuous physical activities for one to two weeks.

Drooping upper eyelids, often a result of aging and a cosmetic concern, can be corrected through a surgical procedure.

How does your treatment at Vista proceed?

During the initial consultation, we will discuss your treatment desires and expectations. After a thorough consultation, we will work together with you to develop a personalized treatment plan tailored to your specific needs. We will schedule the treatment at your convenience.

Upon your request, you can also be treated immediately following the consultation. Depending on the number and size of the treatment areas, the application of most products is straightforward and typically takes around 30 minutes.

To meet your expectations, it requires a dosage and placement of the products listed above that is individually tailored to your needs. To avoid overcorrection, a slight undercorrection is usually intentionally pursued. The result will be complemented in a second session.

The procedures mentioned are extremely gentle, and the products we use have been on the market for many years. Your treating physicians are highly trained and, as experienced surgeons in the field of ophthalmology, they are very familiar with the anatomy of the facial region.

The aesthetic offerings at Vista:

Chemical Peel

A chemical peel involves the use of acid-based substances on the skin. When applied carefully, they can treat conditions such as sun damage, hyperpigmentation, hypopigmentation, as well as thickened skin surfaces.

The chemical peel stimulates collagen production and renews the top layer of the skin, resulting in a firmer complexion.

Cost:
CHF 220 per treatment

Treatment duration:
45-60 minutes

Recovery time:
1 to 6 days

Filler

As we age, our skin loses hyaluronic acid, causing it to become dehydrated and subsequently lose its volume and elasticity. Fillers can counteract this process. Even small amounts of hyaluronic acid can correct fine lines and wrinkles while subtly restoring volume. In addition to addressing individual static wrinkles, they can also be used to successfully shape facial areas.

Following a gentle injection, there is minimal downtime. To achieve a long-lasting result, a second session should take place after several months.

Cost:
CHF 700 per treatment

Upper Eyelid Lift (Blepharoplasty)

The most common cosmetic procedure is the tightening of the upper eyelids when a condition known as «hooded eyelids» is present. With hooded eyelids, the sagging skin touches or even covers the eyelash line. This can result in a feeling of heaviness on the upper eyelids and a restricted field of vision upwards and to the sides. The primary cause is often age-related loss of elasticity.

Hooded eyelids can be surgically corrected through an upper eyelid lift. This procedure involves the removal of excess skin on the upper eyelid.

The surgery does leave a scar, but it is typically well-hidden within the natural eyelid crease. As a result, it is usually no longer noticeable after a few weeks.

The outcome is a more open, alert, and rejuvenated appearance. Additionally, the eyelid is free and can be better suited for makeup application after the procedure.

Local anesthesia with sedation

Recovery time:
1 to 2 weeks

Thread Lift

Starting around the age of thirty, the skin’s tissues begin to sag, gradually forming wrinkles, and altering the facial contour. The reason for this is a change in the supporting structure within the skin. Thread lifting can restore the firmness of the facial skin without surgery. This involves the insertion of fine threads beneath the skin, which stimulates the formation of connective tissue and increases elasticity. The method is also suitable for lifting the eyebrows.

Cost: CHF 950 per treatment

Treatment duration: 35 minutes

Local anesthesia via injection

Recovery time:
1 day

Platelet Rich Plasma (PRP)

PRP is a natural, effective, and long-established procedure in aesthetic medicine. It helps prevent the formation of wrinkles, reduces existing wrinkles, and stimulates the production of connective tissue.

For PRP, a patient’s own blood is drawn and then centrifuged to obtain platelet-rich plasma. This is subsequently injected beneath the skin.

The high concentrations of growth factors in the platelets stimulate cell division and promote skin renewal in the deeper layers.

The result is a sustainable strengthening of the facial skin with an aesthetically rejuvenated appearance.

Cost:
CHF 800 for the initial treatment,
CHF 700 for subsequent treatments

Treatment duration: 45 minutes

Recovery time:
3 to 6 days

Muscle Relaxant

Over the years, our facial muscles undergo stress, resulting in the formation of what are commonly known as frown lines, forehead wrinkles, or crow’s feet, which can make us appear tense.

Muscle relaxants like Botulinum toxin, colloquially referred to as Botox, work by temporarily relaxing the treated facial muscles, smoothing out these so-called expression lines. This effectively treats frown lines, forehead wrinkles, crow’s feet, downturned mouth corners, and eyebrows.

The effect is most visible after one to two weeks, the time it takes for the skin to smoothen. However, the effect is not permanent and needs to be repeated at specific intervals. Regular use can prevent the development of deeper wrinkles altogether.

Cost:
CHF 550 per treatment

Treatment duration:
15 minutes

Skin Rejuvenation with CO2 Laser

A CO2 laser treatment is a precise, gentle, and low-pain method for removing wrinkles, pigment spots, scars, warts, and other benign skin conditions. It can also be used to treat cholesterol deposits.

The laser treatment supports and enhances the skin’s firmness, vitality, and elasticity. Skin cells renew, and new collagen is formed, contributing to wrinkle reduction.

For deeper wrinkles or scars, multiple sessions may be necessary. The result of this clinically proven treatment is noticeably smooth and delicate skin.
